Counsel patients with uterine septum on spontaneous abortion risk: US guidelines

Patients with a septate uterus should be warned of their increased risk of spontaneous abortion and obstetric complications, but there is insufficient evidence of an infertility link, according to new guidance.
The American Society for Reproductive Medicine’s updated guidelines also recommend 3D transvaginal ultrasound as the first-line non-invasive diagnostic tool to assess uterine shape.
While studies on diagnosing uterine septum generally had small sample sizes, the authors said that 3D transvaginal ultrasound with or without saline infusion had “high levels of agreement” with laparoscopy and hysteroscopy for confirmation.
Other diagnostic tools like 2D ultrasound and MRI “may be useful, but are less accurate”, they said.
