Doctors using AI scribes say hallucinations are ‘frequent’: A non-existent neurological exam is just one example

Avant has published a selection of AI scribe errors and 'hallucinations' reported by doctors.

A neurological exam that never took place and a declaration that an oral contraceptive prescription was for a male patient are among the “frequent” AI scribe hallucinations being reported by doctors using AI scribes.

The technology has spread rapidly over the last 12 months, easing routine workload by generating draft medical notes based on doctor–patient conversations during consults.

But doctors attending a recent AI webinar hosted by medical defence organisation Avant have described some of the mistakes.

One incident involved the AI scribe “worryingly … making up a whole neurological exam when I didn’t perform one”.
