Is COVID-19 to blame for this ptosis and bulging cardiac border?

A patient’s right-sided ptosis is believed to be related to a recent SARS-CoV-2 infection until he re-presents with grave symptoms

Ben is a 32-year-old male who presents to ED with a three-day history of intermittent right-sided ptosis.

Ben has a past medical history of ADHD, depression, previous substance misuse disorder (crystal methamphetamine, THC, MDMA and GHB) and narcissistic personality disorder.

He takes no regular medications, is an active smoker (5-6 cigarettes daily) and drinks up to five standard units of alcohol on weekend days. He denies any recent recreational drug use or ever injecting drugs. 

On examination, there is right-sided ptosis, with normal visual acuity and cranial and peripheral nerve examination.
